Fertility can change with weight loss and improved insulin sensitivity. That is a well-described phenomenon and worth knowing about in advance rather than discovering.

A certificate with a purity figure and no method line is not falsifiable. You cannot reproduce it, you cannot check it, and it tells you nothing about how the number was produced.

Formulary criteria are revised, and plan renewal is a natural point at which a previously refused request may succeed without anything about the applicant changing.

Discharge from a specialist service at goal is a normal end point of that pathway. Whether prescribing continues in primary care afterwards is a local decision and is worth asking about at the start rather than at the end.
