comment on the regain question that gets asked weekly, answered properly in c/tapering · 1 points · 7 months ago

Small fix — you have described interval stretching and called it a taper. The exposure profiles differ.

comment on anyone else notice resistance training kicking in around week 92 in c/glp1muscle · 7 points · 8 months ago

Right — strength retention is a free proxy and it tracks better than most people expect.

comment on am I the only one who found resistance training harder than the injections in c/glp1muscle · 0 points · 8 months ago

Compared two scans from different clinics and panicked over a difference that was inside the error margin.

comment on [PSA] prescription is not what most of this community thinks it is in c/glp1eu · 330 points · 8 months ago

Cross-border prescription recognition exists under EU rules but is conditional and narrower than commonly assumed, with requirements about the prescription’s form and content.

DEXA has a meaningful coefficient of variation, and it differs between machines and protocols. Comparing scans from different clinics adds a systematic offset that is invisible on the report.

Supply notices from the regulator describe expected availability at a national level. They do not predict what any individual pharmacy has on the shelf next week.

comment on [Discussion] we are measuring lean mass at the wrong time and calling it noise in c/glp1muscle · 85 points · 11 months ago

Some lean mass loss accompanies weight loss in any deficit. Resistance training and adequate protein intake are the two interventions with the best evidence for limiting the proportion.

the API source is the question nobody asks and everybody should

Adding one thing — ask for the facility name. Most will give it, and the ones that will not have answered you.

a 503B has to register and report, so there is a paper trail to ask for

the pharmacy and the prescriber are two separate questions

permalink

comment on [Discussion] we are measuring regain at the wrong time and calling it noise in c/tapering · 486 points · 1 year ago

With a week-long half-life, exposure declines over several weeks after the last dose rather than stopping with it. The lag is why the first fortnight is uninformative.

That figure is the starting material purity, not the finished preparation potency. Two different tests.

permalink

comment on [Discussion] the GIP advice in here is 2 years out of date in c/tirzepatide · 0 points · 1 year ago

The thing nobody warned me about was how much of this is logistics — storing it, remembering it, the same day every week.

Agreed, with one qualifier: that is the mean of the top arm and the spread around it was enormous.

comment on [Meta] the SURPASS rule is doing its job and people should stop complaining in c/tirzepatide · 1 points · 2 years ago

The distribution matters more than the mean. In the trial population the interquartile spread was wide enough that two honest people can have completely different runs on the same arm.

comment on my fasting insulin moved and I cannot work out whether Zepbound is why in c/tirzepatide · 1k points · 2 years ago

Disagree with the conversion table. There is no validated equivalence between the two molecules and posting one as though there is does real damage.

Correction: that threshold is your ICB’s, not a national one. They differ and they change by year.

Scotland, Wales and Northern Ireland have their own arrangements. An answer that begins with "in England" should say so, and most of the confusion here comes from answers that do not.

Agreed.

Adding the practical bit — ask what happens at discharge before you start, not after.

permalink

comment on [Question] Denmark — has anyone got a straight answer on 503B in c/compounding · 1 points · 2 years ago

Asked for the beyond-use date basis and got a real answer with a stability reference attached. Not universal, apparently.

permalink

comment on genuine question about SURMOUNT that I am slightly embarrassed to ask in c/tirzepatide · 10 points · 2 years ago

Research-use-only material is not approved for human use. Anything in this thread about how a research vial "should" titrate is a discussion about chemistry, not a plan.

permalink

comment on the Mounjaro thing finally clicked for me and I want to write it down in c/tirzepatide · 66 points · 2 years ago

Vials and pens carry the same molecule; what differs is fill volume, device tolerance and whether you are doing your own arithmetic. Neither is inherently more accurate.

comment on [Discussion] can we stop arguing about dual agonist until somebody posts a number in c/tirzepatide · 302 points · 2 years ago

The thing nobody warned me about was how much of this is logistics — storing it, remembering it, the same day every week.

permalink

comment on [Question] how do you actually verify GIP in c/tirzepatide · 170 points · 2 years ago

SURMOUNT-1 ran 72 weeks with the top arm around 21% mean body weight change. SURPASS is the diabetes programme and reports glycaemic endpoints, so quoting the two interchangeably is a category error.
