MACE-3 is not a weight-loss endpoint and that is precisely why SELECT mattered.

comment on [Paper] survodutide MASH data is the most interesting hepatic readout in the class in c/survodutide · 96 points · 1 days ago

a resting HR increase of a few bpm is a consistent class finding and it has not translated into a cardiovascular harm signal in the outcome trials we have. those two facts sit together uncomfortably and both are true.

Correction: that marker is reported in different units by different labs, which explains the tenfold difference you are seeing.

Careful — that arithmetic is wrong. 10mg in 3mL is 3.33mg/mL, so 2.5mg draws to 75 units, not what you wrote.

Escalating faster than the schedule allows is the most frequently described regret on the site, and the mechanism is straightforward: tolerance is the limiting factor and it does not care about impatience.

Supply notices are issued nationally. A notice in one member state does not imply availability problems in another, even for the same product.

If your calculation produces a fraction of a unit, the concentration is wrong for your dose. Change the water volume until the numbers land on whole units.

Employer-sponsored plans may be regulated differently from individually purchased ones, which changes both the appeal route and which regulator hears a complaint.

Most of what gets posted here is procedural rather than dramatic: a step skipped, a document not requested, a temperature not considered, a pace chosen out of impatience.

comment on how much of what we believe about mg/mL actually comes from reconstitution threads in c/reconstitution · 218 points · 1 months ago

Why "more water" keeps confusing people, including me for a month.

The vial contains a fixed amount of peptide. Water changes nothing about that amount. What it changes is concentration, and concentration is the divisor between the dose you want and the volume you draw.

So doubling the water halves the concentration and doubles the units for the same dose. Nothing is diluted in any sense that matters; you are simply reading a bigger number off the same barrel. Pick the water volume that puts your usual dose on a whole number of units and life gets easier.

comment on [Meta] the units rule is doing its job and people should stop complaining in c/reconstitution · 13 points · 2 months ago

Aim the stream down the wall of the vial rather than into the cake. Reconstitution is a solubility problem, not a mixing problem, and mechanical shear does nothing helpful.

comment on [Discussion] can we stop arguing about copay until somebody posts a number in c/insurancefights · 1 points · 2 months ago

Why step therapy denials feel unfair and are nonetheless beatable.

The requirement is that documented trials of preferred alternatives exist. It is a record-keeping standard, not a clinical judgement about you, which is why arguing the clinical merits rarely moves it and producing dates and durations often does.

What to assemble: what was tried, at what dose, for how long, and what the documented outcome was. A one-page table with dates beats three pages of prose every time. Where a trial is contraindicated rather than simply unsuccessful, that needs to be stated explicitly by the prescriber in those terms.

None of this is legal or medical advice — it is what the threads here have found works.

Spent six weeks arguing in general terms and got nowhere. Two paragraphs quoting their own criteria turned it around in eleven days.

comment on how much of what we believe about mg/mL actually comes from insulin syringe threads in c/reconstitution · 90 points · 2 months ago

The failure modes I have actually seen posted here, in order of frequency.

One: reconstituting at a different volume than intended and not writing it down, so the next draw is based on a remembered concentration. Two: treating units as a dose rather than a volume, which breaks the moment the concentration changes. Three: assuming a syringe is U-100 when it is not.

All three are solved by the same two habits — do the arithmetic before the water goes in, and write the result on the glass. Neither costs anything and between them they cover almost every question this board gets.

Correction: that gives 10 units, not what you have written. 30mg into 2mL is 15mg/mL and the division follows from there.
