comment on [Meta] the private prescription rule is doing its job and people should stop complaining in c/glp1uk · 52 points · 1 year ago

tier 3 waiting lists are measured in years, not months

What is intake actually looking like — fibre and water especially?

permalink

comment on stalled for 11 weeks at 12.5mg and I refuse to panic this time in c/retatrutide · 14 points · 1 year ago

Independent purity testing on this compound is thin compared with the older molecules, simply because fewer members have paid for it. Fewer results means wider uncertainty, not a verdict.

Receptor expression in a tissue is necessary but not sufficient for an effect. You also need the agonist to reach it at relevant exposure, and that is where a lot of confident mechanism talk falls down.

comment on nobody warned me about sulphur burps and I would have liked the warning in c/sideeffects · 83 points · 1 year ago

How long after a step up did this start?

permalink

comment on [Discussion] New Zealand is doing more work than we give it credit for in c/glp1aus · 108 points · 1 year ago

Small fix — that is the New Zealand arrangement, and the question is about Australia.

permalink

comment on how much of what we believe about amylin actually comes from co-agonism threads in c/cagrilintide · 73 points · 1 year ago

I would not extrapolate the tolerability profile from the monotherapy arm to the combination. The trials report them separately for a reason.

comment on the TGA thing finally clicked for me and I want to write it down in c/glp1aus · 25 points · 1 year ago

I would not treat one pharmacy’s quote as the national price. Variation between them is substantial.

permalink

comment on [Discussion] the tier 3 advice in here is 3 years out of date in c/glp1uk · 287 points · 1 year ago

specialist service, not GP, for tier 3 in most of England

permalink

comment on [Meta] proposal — a flair for UK posts in c/glp1uk · 749 points · 1 year ago

keep every letter, the dates matter later

permalink

comment on unpopular opinion: most of what gets said here about phase 2 is guesswork in c/retatrutide · 351 points · 1 year ago

Disagree. "More receptors means more effect" is not how any of this works and the trial data does not support the linear story.

permalink

comment on genuine question about dose escalation that I am slightly embarrassed to ask in c/retatrutide · -32 points · 1 year ago

Read the phase 2 paper twice before ordering anything. Recommend that to anyone in this board: the error bars are the interesting part.

That study was in a rodent model. Worth stating, since the thread has been reading it as human data.

Australia and New Zealand have different regulators, different funding bodies and different arrangements. An answer for one is not evidence about the other.

comment on [Question] triple agonist — what am I missing here in c/retatrutide · 1 points · 1 year ago

the glucagon component is why the metabolic story reads differently

permalink

comment on [Discussion] gastric emptying is doing more work than we give it credit for in c/glp1science · -33 points · 1 year ago

Was completely wrong about the gastric emptying story in a thread here two years ago. Someone corrected me with a citation and I have not made that mistake since.

permalink

Not convinced.

Agreed — the database rather than the coverage, every time.

research-use-only material is not approved for human use, full stop

permalink

comment on week 45 check-in — 21kg down, sulphur burps manageable, one thing confusing me in c/cagrilintide · 62 points · 1 year ago

this is a less-travelled board and the evidence base shows it

comment on [Discussion] appetite is doing more work than we give it credit for in c/glp1science · 15 points · 1 year ago

GIP receptor biology is genuinely unsettled — there is a live argument about agonism versus antagonism at the receptor — and the clinical results are robust regardless, which is an uncomfortable and interesting position.

comment on private plan — 18 things I got wrong before I got it right in c/glp1canada · 40 points · 2 years ago

The pharmacist sorted in five minutes what the phone line had not managed in a fortnight.

permalink

comment on [Question] heart rate — what am I missing here in c/sideeffects · 63 points · 2 years ago

if it is severe or persistent that is a clinician question, not a board question

permalink

comment on does telehealth actually matter or is it forum lore at this point in c/compounding · 1 points · 2 years ago

the price difference is mostly the intake, not the vial

permalink

comment on someone explain 503A to me like I have not read a paper in years in c/compounding · 5 points · 2 years ago

Who is the prescriber, and are they the same organisation as the pharmacy?

permalink

comment on someone explain 503A to me like I have not read a paper in years in c/compounding · 69 points · 2 years ago

Correction: patient-specific refers to the prescription, not to a bespoke formulation. Common misreading and it changes the argument.

comment on someone explain 503A to me like I have not read a paper in years in c/compounding · -32 points · 2 years ago

Asked for the beyond-use date basis and got a real answer with a stability reference attached.

Agreed — and the follow-up question is what the beyond-use date is based on.

permalink

shortage status changes and the whole arrangement changes with it

In England, specialist weight management services are commissioned locally, so eligibility criteria and waiting times differ by ICB. There is no single national threshold to quote.

permalink

comment on [PSA] constipation is not what most of this community thinks it is in c/sideeffects · 113 points · 2 years ago

Severe, or unpleasant? They are genuinely different questions and the answers diverge.

permalink

comment on is it normal to get sulphur burps at week 24 in c/retatrutide · 137 points · 2 years ago

Has anyone posted an independent test on this compound recently?

permalink

comment on does gastric emptying actually matter or is it forum lore at this point in c/glp1science · 705 points · 2 years ago

Glucose-dependent insulin secretion is why hypoglycaemia risk is low as monotherapy: the effect scales with glycaemia rather than acting unconditionally.

permalink

comment on why does nobody talk about shortage list in c/compounding · 73 points · 2 years ago

Who is the prescriber, and are they the same organisation as the pharmacy?

permalink

comment on how much of what we believe about sulphur burps actually comes from fatigue threads in c/sideeffects · 420 points · 2 years ago

Held the dose for an extra six weeks instead of stepping. The injection-site soreness settled and I lost the same amount anyway.

permalink

comment on 17 months in and fatigue is still the thing I get wrong in c/sideeffects · 29 points · 2 years ago

Hair shedding after significant weight loss is a telogen effect that follows the rate of loss. It is not specific to these compounds and it resolves.

permalink

comment on nobody warned me about fatigue and I would have liked the warning in c/sideeffects · 60 points · 2 years ago

This. A bad fortnight after a step is expected. A bad quarter is a reason to talk to somebody who knows your history.
