I would not post that as a finding. It is a single result and the template asks you to say so.

private and NHS pathways are not a ladder, they are separate routes

Discharge from a specialist service at goal is a normal end point of that pathway. Whether prescribing continues in primary care afterwards is a local decision and is worth asking about at the start rather than at the end.

Nobody here can interpret your panel and this comment is not doing so. What a board can usefully do is help you ask better questions of somebody who can.

Yes — write the concentration in the log as well as on the vial. Belt and braces and it costs nothing.

Yes — date of analysis, because a result from two years ago describes a different manufacturing month.

That is a sensor value, not a plasma value, and the two are not interchangeable at that level of precision.

Inter-lab variance on this assay runs roughly a point either way, driven mostly by gradient and integration. Two results inside that band agree; they do not disagree quietly.

Correction: that threshold is your ICB’s, not a national one. They differ and they change by year.
