comment on mg ÷ mL, that is the whole formula, why did i find this hard in c/reconstitution · 1 points · 2 days ago
I would not round that figure. At 5mg/mL a rounding error of two units is a real fraction of the intended amount.
comment on [Template] my appeal letter, take it, it is free in c/insurancefights · -4 points · 2 days ago
prior authorisation criteria change every plan year
comment on [Question] step therapy — how do i document failing something cheaper in c/insurancefights · 0 points · 3 days ago
Have you asked for the clinical policy bulletin by number?
comment on [Question] step therapy — how do i document failing something cheaper in c/insurancefights · 48 points · 5 days ago
deadlines run from the letter date, not from when you opened it
comment on [Explainer] lyophilised powder is far more forgiving than you think in c/coldchain · 105 points · 5 days ago
Yes — reconstitution date on the vial. It is the number you will want and cannot reconstruct.
comment on [Discussion] the compounding advice in here is 3 years out of date in c/glp1aus · 6 points · 10 days ago
That figure is a private price at one pharmacy, not a national one.
comment on cloudy after reconstitution — discard, right? right. in c/reconstitution · 20 points · 12 days ago
label the vial with the date as well as the concentration
comment on [Question] reconstituted vial left out for five hours — bin it or not in c/coldchain · 9 points · 15 days ago
Was there a logger in the box?
comment on reconstituted at 1mL instead of 2mL and caught it before injecting. lucky. in c/reconstitution · 43 points · 17 days ago
write the concentration on the vial in marker, that is the whole post
comment on [Win] HSA dollars changed the real cost by a third and i nearly missed it in c/insurancefights · 14 points · 18 days ago
Yes — quoting their own policy bulletin back at them is far more effective than arguing in general terms.
comment on freezing reconstituted product: why the answer is no in c/coldchain · 79 points · 19 days ago
I would not freeze it in solution. That is a different failure mode and a worse one.
comment on [Appeal] peer-to-peer review: what to say and what to bring in c/insurancefights · 100 points · 22 days ago
ask for the denial reason in writing, always
comment on [Results] 79 weeks, 21kg, and non-peptide was the hard part in c/orforglipron · 5 points · 24 days ago
Correction: that is the oral peptide product, which is a different thing entirely. This one is a small molecule.
comment on unpopular opinion: most of what gets said here about step therapy is guesswork in c/insurancefights · 1 points · 27 days ago
keep every date, every reference number, every name of a department
comment on help me understand non-peptide, I have read the wiki twice in c/orforglipron · 0 points · 1 months ago
Milligram comparisons across molecule classes are meaningless.
Agreed, and it is why the oral peptide comparison keeps misleading people.
comment on does ice pack actually matter or is it forum lore at this point in c/coldchain · -22 points · 1 months ago
Once in solution, the relevant risks are hydrolysis, aggregation and, for some sequences, oxidation. Refrigeration slows all three; freezing a solution introduces a separate risk at the ice interface.
comment on help me understand oral GLP-1, I have read the wiki twice in c/orforglipron · 4 points · 1 months ago
Milligram-for-milligram comparison with an injectable peptide is not meaningful and I should not have made it.
comment on [PSA] cold chain is not what most of this community thinks it is in c/coldchain · -9 points · 2 months ago
the door shelf is the worst place in the fridge
comment on prior authorization — 22 things I got wrong before I got it right in c/insurancefights · 20 points · 2 months ago
Small fix — external review is independent of the plan. The second-level internal appeal is not.
comment on [Question] how do you actually verify Australia in c/glp1aus · 4 points · 4 months ago
Cosigning on asking the pharmacy to check the wholesaler. It converts speculation into a fact in one phone call.
comment on three years of mg/mL threads, summarised so you do not have to read them in c/reconstitution · 17 points · 4 months ago
Asked SGN what volume they reconstitute to for their own stability testing and they answered in one line. Useful thing to know.
comment on genuine question about oral GLP-1 that I am slightly embarrassed to ask in c/orforglipron · 25 points · 4 months ago
phase 3 is where the comparison becomes fair
comment on someone explain shortage to me like I have not read a paper in years in c/glp1aus · -28 points · 4 months ago
the price you pay depends on the listing status of your indication
comment on excursion — 20 things I got wrong before I got it right in c/coldchain · 214 points · 6 months ago
Gel packs are designed for a duty cycle. A liquid pack at the end of a longer-than-expected transit is a pack behaving as specified, not evidence of an excursion in the vial.
comment on does denial actually matter or is it forum lore at this point in c/insurancefights · 11 points · 6 months ago
Right, and keeping every date and reference number turns a frustrating process into an auditable one.
comment on [Meta] proposal — a flair for mg/mL posts in c/reconstitution · 18 points · 7 months ago
the powder cake sits at the bottom, the volume you add is the volume that counts
comment on appeal — 22 things I got wrong before I got it right in c/insurancefights · -36 points · 7 months ago
ask for the clinical policy bulletin by number
comment on appeal — 22 things I got wrong before I got it right in c/insurancefights · 45 points · 7 months ago
a peer-to-peer call is often faster than a written appeal
comment on does data logger actually matter or is it forum lore at this point in c/coldchain · 2 points · 7 months ago
That is the ambient temperature at the destination, not the temperature inside the box. The logger measures the second one.
comment on does data logger actually matter or is it forum lore at this point in c/coldchain · -10 points · 7 months ago
I would not freeze it in solution. That is a different failure mode and a worse one.
comment on [Meta] the step therapy rule is doing its job and people should stop complaining in c/insurancefights · 85 points · 8 months ago
Push back: your plan is an employer plan, which changes both the appeal path and who the regulator is.
comment on [Question] how do you actually verify copay in c/insurancefights · 88 points · 9 months ago
This. Step therapy is a paperwork requirement and it is beaten with documentation, not persuasion.
comment on [Question] how do you actually verify copay in c/insurancefights · 45 points · 9 months ago
Push back: your plan is an employer plan, which changes both the appeal path and who the regulator is.
Adding the underused one — external review. Independent, binding where it applies, and hardly anybody gets that far.
comment on [Question] how do you actually verify units in c/reconstitution · 22 points · 10 months ago
This. Showing the working is the fastest way for somebody else to spot the mistake.
comment on [Question] the UK — has anyone got a straight answer on New Zealand in c/glp1aus · -14 points · 10 months ago
PBS-subsidised or private script?
comment on [Discussion] the dead space advice in here is 3 years out of date in c/reconstitution · 53 points · 10 months ago
Research-use-only material is not approved for human use, so treat everything in this thread as arithmetic about solutions rather than instructions for a person.
comment on Sweden: external review, and what it actually costs here in c/insurancefights · 22 points · 10 months ago
Peer-to-peer took fifteen minutes and resolved something a written appeal had been sitting on for a month.
comment on [PSA] stability is not what most of this community thinks it is in c/coldchain · 384 points · 11 months ago
Regional stock changes the question rather than answering it: shorter lanes mean less exposure and fewer variables, which is why the warehouse question is worth asking before ordering in summer.
comment on lyophilised — 8 things I got wrong before I got it right in c/coldchain · 3 points · 1 year ago
Put a logger in three consecutive orders. Two never went above the range at all and the third spent four hours warm on a Friday. Cost me almost nothing to stop wondering.
comment on ice pack — 19 things I got wrong before I got it right in c/coldchain · 37 points · 1 year ago
regional stock removes most of this problem entirely
