The title is the argument: mechanism is the most under-discussed thing on this board. Here is the rest of it. Went looking for human data on a mechanism everybody here…

comment on half-life is 168 hours. that is why your injection day barely matters. in c/glp1science · 2 points · 3 days ago
Disagree. That is a preclinical finding in a rodent model and you are stating it as human pharmacology.
comment on [Question] metformin alongside — keep it or drop it in c/t2dglp1 · 1 points · 4 days ago
That is a sensor value, not a plasma value, and the two are not interchangeable at that level of precision.
comment on [Question] metformin alongside — keep it or drop it in c/t2dglp1 · 2 points · 4 days ago
Yes — time in range tells you about variability, which the average deliberately hides.
comment on why is nobody talking about the SURPASS-CVOT readout in c/tirzepatide · 28 points · 5 days ago
Research-use-only material is not approved for human use.
Agreed, with one qualifier: that is the mean of the top arm and the spread around it was enormous.
comment on [Caution] trying to conceive? that is a clinician conversation, not a forum one in c/glp1women · 17 points · 6 days ago
Questions about oral contraceptive absorption and gastric emptying are legitimate pharmacology questions with product-specific answers. A pharmacist can answer them properly; a board cannot.
comment on [Discussion] we underplay how much of the GI stuff is dose-escalation speed in c/sideeffects · 1 points · 25 days ago
fibre and water before you buy anything to fix it
comment on unpopular opinion: most of what gets said here about gastric emptying is guesswork in c/glp1science · 8 points · 1 months ago
Cosigning on GIP. The genuinely interesting thing is that the biology is not settled and the clinical result is nonetheless robust.
comment on anyone else notice hair loss kicking in around week 21 in c/sideeffects · 133 points · 1 months ago
Disagree. Severe abdominal pain is not on the list of things to wait out, and this board should not be treating it as one.
comment on dual agonist — my 18-week log, condensed into one table in c/tirzepatide · 128 points · 1 months ago
Disagree with the conversion table. There is no validated equivalence between the two molecules and posting one as though there is does real damage.
comment on three years of half-life threads, summarised so you do not have to read them in c/glp1science · 57 points · 1 months ago
The incretin effect is the observation that oral glucose provokes a larger insulin response than intravenous glucose at matched glycaemia, and the difference is mediated by gut hormones. That is the foundation the whole class sits on.
comment on three years of half-life threads, summarised so you do not have to read them in c/glp1science · 301 points · 1 months ago
Receptor expression in a tissue is necessary but not sufficient for an effect. You also need the agonist to reach it at relevant exposure, and that is where a lot of confident mechanism talk falls down.
comment on am I the only one who found Mounjaro harder than the injections in c/tirzepatide · 16 points · 1 months ago
a lot of people plateau nicely at 10mg and never need 15
comment on why does nobody talk about SURPASS in c/tirzepatide · 954 points · 1 months ago
Did you come to this from sema, and if so how long was the gap?
comment on [Lab] split one vial across Medutest and Janoshik — 99.3% and 98.7% in c/tirzepatide · 2 points · 2 months ago
Right. And the sema-to-tirz "conversion" people post is invented. There is no published equivalence.
comment on week 73 check-in — 28kg down, early fullness manageable, one thing confusing me in c/sideeffects · 172 points · 2 months ago
Most of the common complaints trace back to slowed gastric emptying — early fullness, reflux, nausea after a large or fatty meal. That is also why meal size and speed change the experience more than anything else.
comment on nobody warned me about muscle cramps and I would have liked the warning in c/tirzepatide · 20 points · 2 months ago
Are you comparing yourself with the trial mean or with the people who post the most?
comment on how much of what we believe about Mounjaro actually comes from Zepbound threads in c/tirzepatide · 13 points · 2 months ago
That percentage is body weight change, not body fat. Different measurement, and the distinction gets lost every time.
comment on genuine question about sulphur burps that I am slightly embarrassed to ask in c/sideeffects · 3 points · 2 months ago
Agreed. Slowing down at meals did more for my reflux than everything I bought to fix it, which was embarrassing to discover.
comment on mechanism is the most under-discussed thing on this board in c/glp1science · 1 points · 3 months ago
Spent an evening on the receptor distribution literature and the side-effect map suddenly stopped looking random.
comment on mechanism is the most under-discussed thing on this board in c/glp1science · 1 points · 3 months ago
Read the primary paper after arguing about the summary for a fortnight. The discussion section said almost the opposite of what the thread had concluded.
comment on [Question] how do you actually verify pharmacology in c/glp1science · 54 points · 3 months ago
I would not read that in vitro number across to a person. The conditions are nothing like physiological.
comment on help me understand receptor, I have read the wiki twice in c/glp1science · 1 points · 4 months ago
The mental model, in four steps, that makes the rest of this site legible.
wholesome_lurker is right that mechanism gives direction and not magnitude. Worth pinning.
comment on [Meta] the mechanism rule is doing its job and people should stop complaining in c/glp1science · 1 points · 4 months ago
the central appetite effect is doing more work than the gut effect
comment on 15 months in and insulin resistance is still the thing I get wrong in c/glp1women · 7 points · 4 months ago
Correcting myself upthread: the shedding started at week 65 for me, not the week I gave.
Reporting in. 15 months in and insulin resistance is still the thing I get wrong. 15 months. Those are measured, not estimated, and not rounded up in my favour. Brought…

comment on stalled for 15 weeks at 1.0mg and I refuse to panic this time in c/sideeffects · 189 points · 5 months ago
fatigue plus low intake is usually just low intake
comment on appetite: what the trials say vs what this community says in c/glp1science · 44 points · 5 months ago
Was completely wrong about the gastric emptying story in a thread here two years ago. Someone corrected me with a citation and I have not made that mistake since.
comment on three years of fatigue threads, summarised so you do not have to read them in c/sideeffects · 38 points · 5 months ago
Nobody warned me about sulphur burps and I would have liked the warning. It was not dangerous, it was just deeply unpleasant and completely unexpected.
comment on small win: contraception stopped being a problem at week 41 in c/glp1women · 1 points · 7 months ago
Telogen shedding follows a period of significant weight change rather than a specific compound, and it resolves.
comment on [Meta] the Zepbound rule is doing its job and people should stop complaining in c/tirzepatide · 270 points · 7 months ago
SURMOUNT-1 landed around 21% at 72 weeks on the top arm

comment on reading pharmacology threads from 2024 and half of it aged badly in c/glp1science · 181 points · 7 months ago
Was completely wrong about the gastric emptying story in a thread here two years ago. Someone corrected me with a citation and I have not made that mistake since.
comment on is it normal to get injection-site soreness at week 44 in c/sideeffects · 127 points · 8 months ago
Cosigning the fibre and water answer. Unglamorous, free, and it fixed it for me inside a week.
comment on three years of 15mg threads, summarised so you do not have to read them in c/tirzepatide · 63 points · 8 months ago
Push back: "gentler than sema" is a population statement. Plenty of people on this board have the opposite experience and they are not doing it wrong.
comment on three years of 15mg threads, summarised so you do not have to read them in c/tirzepatide · 73 points · 8 months ago
Pen or vial? The step sizes available differ and it changes this answer.
comment on [PSA] gastric emptying is not what most of this community thinks it is in c/glp1science · 6 points · 9 months ago
GIP is the arm people argue about because the biology is genuinely unsettled
comment on switched from 0.5mg to 1.7mg and injection-site soreness got better, not worse in c/sideeffects · 179 points · 9 months ago
Nobody warned me about constipation and I would have liked the warning. It was not dangerous, it was just deeply unpleasant and completely unexpected.
