Somebody posts a proposal with replacement wording — not a complaint, actual text. It gets argued about, usually for a week or two. If it survives, it goes in with whatever amendments came out of the argument, and the change is announced with the reasoning attached.

Different endpoints: body weight change, glycaemic control and cardiovascular events are not convertible. Different durations: 68 weeks and 72 weeks are not the same, and the curves have not flattened by either.

Different analysis populations: one paper reports intention-to-treat, another emphasises completers. Different support: some trial designs include structured lifestyle contact that no member of this board receives.

Stack those and the "X beats Y" tables that circulate here are comparing five things at once and attributing the difference to the molecule.

The requirement is that documented trials of preferred alternatives exist. It is a record-keeping standard, not a clinical judgement about you, which is why arguing the clinical merits rarely moves it and producing dates and durations often does.

What to assemble: what was tried, at what dose, for how long, and what the documented outcome was. A one-page table with dates beats three pages of prose every time. Where a trial is contraindicated rather than simply unsuccessful, that needs to be stated explicitly by the prescriber in those terms.

A compounded vial can be filled at a different concentration from the branded product you were previously using. If you carry over your old arithmetic — same number of units, different mg/mL — you draw a different amount and may not notice for weeks.

Read the label every time you switch. Redo the arithmetic on paper. Write the concentration on the vial. It is the same habit c/reconstitution keeps preaching and this is the case where it actually bites.
