this is basically my whole reasoning too, wasn't chasing an outcome-data-backed protocol, was trying to solve "the jump from here to there is too big for my stomach" with the tools that exist

this is exactly what I ended up doing around month 3, reconstituted more concentrated once I noticed the dead-space math getting annoying at the smallest steps

doing the arithmetic on why this feels so bad: going from 2.5mg to 5mg is a 100% jump in one step. going 2.5 → 3.75 → 5 with an extra rung is two ~50% jumps instead of one 100% jump. same destination, half the shock per step, that's the whole rationale for the smaller bumps.

Why so many single values look alarming and turn out to be nothing.

A reference interval is built to contain about 95% of a healthy reference population, so one test in twenty falls outside one by construction. Add biological variation, assay imprecision, fasting state and time of day, and a genuinely stable person will produce occasional out-of-range results.

That is why repeat testing before acting is standard. Regression to the mean handles most of it. None of which means an out-of-range value should be ignored — it means it should be repeated and taken to somebody who can put it in context, which is emphatically not a ranked feed.

What did the repeat show?

Steady state after a dose change takes four to five half-lives, so the fair judgement point on a new step is about a month, not four days.

Small fix — 214nm, not 210. It matters for the comparison you are making with the other run.

The escalation schedule in the trials was designed to get people to a fixed study dose. Real use is not a trial. The dose that keeps your appetite quiet with side effects you can live with is the dose, and for a lot of people on this board that has been 0.5mg for a very long time.

The counter-argument, which is fair: some people genuinely do need the top of the range, and staying low out of caution costs them months. That is a conversation with someone who knows your history, not with us.

I would not extrapolate the tolerability profile from the monotherapy arm to the combination. The trials report them separately for a reason.

Same lane, four orders, 13 to 11 days, all four within a couple of days of the quote. That consistency is the actual finding.

I would not blame the sample yet. Everything you have described is consistent with the column rather than the vial.
