The four things a certificate must carry, and what each one is for.
Batch identifier ties the document to your vial; without it the document is about somebody else’s material. Date of analysis tells you when the number was true. Method — column, gradient, wavelength — is what makes the figure reproducible rather than asserted. Individual related substances give you the shape of what else is in there.
Everything beyond those four is welcome. Anything missing from them is a question to ask before you order, and in my experience the answer arrives quickly from the suppliers who have a batch record to draw it from.

The arithmetic in full: concentration is mass over volume, so 20mg into 2mL gives 10mg/mL.
Right, and the arithmetic behind it: 15mg in 3mL is 5mg/mL, so 0.75mg is 15 units.

The whole calculation, once, with real numbers.
You have a 10mg vial. You add 1mL of bacteriostatic water. Concentration is mass over volume: 10mg/mL. A U-100 insulin syringe is graduated at 100 units per millilitre, so one unit is 0.01mL.
To draw 5mg you need 5 divided by 10 millilitres, which is 50 units on the barrel. Write "10mg/mL" on the vial in marker before you put it away, and you never have to do this again for that vial.

Why so many single values look alarming and turn out to be nothing.
A reference interval is built to contain about 95% of a healthy reference population, so one test in twenty falls outside one by construction. Add biological variation, assay imprecision, fasting state and time of day, and a genuinely stable person will produce occasional out-of-range results.
That is why repeat testing before acting is standard. Regression to the mean handles most of it. None of which means an out-of-range value should be ignored — it means it should be repeated and taken to somebody who can put it in context, which is emphatically not a ranked feed.

Not quite. Bacteriostatic and sterile water are not interchangeable for a multi-draw vial, and the difference is the preservative.

What actually matters, in order, because these threads always start at the wrong end.
State first: dry powder is comparatively robust, reconstituted solution is not. Most of the anxiety here is about powder and most of the real risk is about solution.
Then evidence: a warm gel pack is evidence about the pack. A logger is evidence about the box. Only a test is evidence about the material. People routinely reason from the first to the third and it does not follow.
Then prevention, which is dull and effective: shorter lanes where regional stock exists, rigid packaging, the middle of the fridge rather than the door, and the reconstitution date written on the glass.
