comment on [PSA] stability is not what most of this community thinks it is in c/coldchain · 384 points · 11 months ago

Regional stock changes the question rather than answering it: shorter lanes mean less exposure and fewer variables, which is why the warehouse question is worth asking before ordering in summer.

Put a logger in three consecutive orders. Two never went above the range at all and the third spent four hours warm on a Friday. Cost me almost nothing to stop wondering.

regional stock removes most of this problem entirely

Agreed. The dry powder is the robust state; it is the solution that needs the care.

more water is not more drug, it is the same drug in a bigger volume

Agreed on the swirl. There is no reason to shake it and a good reason not to.

Lyophilisation removes the water that drives most degradation pathways. That is why the dry cake is comparatively robust and the reconstituted solution is not.

do the arithmetic before you reconstitute, not while holding a syringe

permalink

comment on the prior authorization question that gets asked weekly, answered properly in c/insurancefights · 11 points · 1 year ago

Not convinced. That is a formulary exclusion rather than a prior authorisation denial, and the route to challenge it is different.

Milligram-for-milligram comparison with an injectable peptide is not meaningful and I should not have made it.

comment on Germany: data logger, and what it actually costs here in c/coldchain · 16 points · 1 year ago

Split an order across two months deliberately to compare summer and winter transit on the same lane. Four days difference, same carrier.

comment on unpopular opinion: most of what gets said here about orforglipron is guesswork in c/orforglipron · 8 points · 1 year ago

Daily dosing gives a very different exposure profile from a weekly injectable: peaks and troughs within each day rather than a smoothed weekly curve.

yara_boateng is right that milligram comparisons across classes tell you nothing.

comment on [Question] reconstitution — what am I missing here in c/reconstitution · 4 points · 1 year ago

Adding more water does not change how much peptide is in the vial. It changes the concentration, which changes the volume you draw for the same amount. That is the entire relationship.

Agreed on the swirl. There is no reason to shake it and a good reason not to.

comment on [Vendor] WXT order #15 — Germany, 12 days, no drama, receipts inside in c/coldchain · 1 points · 1 year ago

That is the ambient temperature at the destination, not the temperature inside the box. The logger measures the second one.

a warm gel pack tells you the pack failed, not that the material did

dead space in the needle hub is real and it is small

comment on three years of compounding threads, summarised so you do not have to read them in c/glp1aus · 84 points · 1 year ago

I would not treat one pharmacy’s quote as the national price. Variation between them is substantial.

write the concentration on the vial in marker, that is the whole post

I would not skip the peer-to-peer. It is often the fastest route and it costs a phone call.

the diagnosis code on the claim is doing more work than anything you write

Spent an evening reading about small-molecule agonism at a peptide receptor. Genuinely interesting engineering.

comment on unpopular opinion: most of what gets said here about denial is guesswork in c/insurancefights · 302 points · 2 years ago

A denial letter is required to state a reason and to reference the criterion applied. That reference is the handle: request the clinical policy document by its identifier and answer it point by point.

That advice is jurisdiction-specific and this board spans several. Say where you are.

comment on [Question] how do you actually verify insulin syringe in c/reconstitution · 56 points · 2 years ago

That is sterile water, not bacteriostatic. Same appearance, different product, and it matters for a multi-draw vial.

I keep a laminated card in the drawer with the arithmetic on it. Looks ridiculous. Has never once let me draw the wrong volume.

comment on [Discussion] data logger is doing more work than we give it credit for in c/coldchain · 42 points · 2 years ago

Which lane, and had it been quoted at that transit time?

permalink

comment on reading concentration threads from 2024 and half of it aged badly in c/reconstitution · 181 points · 2 years ago

If your calculation produces a fraction of a unit, the concentration is wrong for your dose. Change the water volume until the numbers land on whole units.
