comment on [Question] how do you actually verify 503B in c/compounding · -22 points · 1 year ago

Asked which facility and got a name straight away. Looked it up, found the registration, felt considerably better about the whole thing.

It is a dual GIP and GLP-1 receptor agonist, and the GIP arm is the part that has no equivalent in the semaglutide threads.

Not sure about this bit. The GIP arm being real does not tell you that it is what caused your particular week.

Weekly dosing again, half-life in the same neighbourhood as sema, so a step change takes about a month to reach steady state. Judging a new step at day five is judging noise.

What is the waist doing? That is usually the number still moving during a scale stall.

Was there potency testing on the finished preparation, or only on the starting material?

Split one order across two months on purpose to compare summer and winter. Four days apart, same carrier, same route.

The rigid outer with no rattle in it was the thing that told me most about that supplier, before I had tested anything.

Nothing containing this compound is approved anywhere.

This is the framing the board needs. It is a hepatic programme first.

Same view. A record you can read beats a rating you cannot inspect, every time.

Right. And the sema-to-tirz "conversion" people post is invented. There is no published equivalence.

The wider-market rows in the directory carry a band and a report count and nothing else, deliberately. Anything more would be manufacturing precision that the evidence does not support.

Correction to my own post above — I said 7.5mg and I have been on 5mg since the spring. Same argument, wrong number.

Week 72 was the first time the scale moved after a five-week stall. I changed nothing in that window.

the number you write down is the number you will trust at 6am

Disagreeing with this line only: adding more water is not a safety margin, it is a different divisor.

Correction: patient-specific refers to the prescription, not to a bespoke formulation. Common misreading and it changes the argument.

comment on [Discussion] dual agonist is doing more work than we give it credit for in c/tirzepatide · 63 points · 1 year ago

The four-week interval in the label is a minimum. Nothing about the pharmacology requires you to step on schedule, and the trials stepped on a calendar because trials have to.

permalink

comment on anyone else notice GIP kicking in around week 81 in c/tirzepatide · 1 points · 1 year ago

Cosigning the four-week thing. It is a minimum interval, and treating it as a schedule to keep up with is how people end up miserable at 12.5.

I keep a laminated card in the drawer with the arithmetic on it. Looks ridiculous. Has never once let me draw the wrong volume.

Independent purity results for this compound are very sparse across the whole site, which means any impression about consistency is built on almost nothing.

permalink

comment on [Question] how do you actually verify mg/mL in c/reconstitution · 50 points · 2 years ago

Correction: that gives 30 units, not what you have written. 15mg into 3mL is 5mg/mL and the division follows from there.

If your calculation produces a fraction of a unit, the concentration is wrong for your dose. Change the water volume until the numbers land on whole units.

Careful with "everyone tolerates it better". The people it did not suit stop posting, which makes this board look calmer than the drug is.

Did the maths give you a whole number of units, or a fraction?

permalink

comment on unpopular opinion: most of what gets said here about concentration is guesswork in c/reconstitution · -32 points · 2 years ago

What concentration did you actually reconstitute at, rather than what did you intend to?

permalink

comment on unpopular opinion: most of what gets said here about concentration is guesswork in c/reconstitution · 87 points · 2 years ago

Small fix — a U-100 syringe is 100 units per mL. If yours is marked differently it is not U-100 and the whole calculation changes.

permalink

comment on bacteriostatic water — 16 things I got wrong before I got it right in c/reconstitution · 79 points · 2 years ago

Not quite. Bacteriostatic and sterile water are not interchangeable for a multi-draw vial, and the difference is the preservative.

permalink

comment on bacteriostatic water — 16 things I got wrong before I got it right in c/reconstitution · 191 points · 2 years ago

Bacteriostatic water contains benzyl alcohol as a preservative, which is what makes repeated entry into the same vial reasonable. Plain sterile water has no preservative.

Careful — that arithmetic is wrong. 20mg in 2mL is 10mg/mL, so 5mg draws to 50 units, not what you wrote.

comment on [Discussion] we are measuring GIP at the wrong time and calling it noise in c/tirzepatide · 938 points · 2 years ago

SURMOUNT-1 ran 72 weeks with the top arm around 21% mean body weight change. SURPASS is the diabetes programme and reports glycaemic endpoints, so quoting the two interchangeably is a category error.
