comment on [NHS] two years and four months on a tier 3 waiting list. timeline inside. in c/glp1uk · 9 points · 1 days ago
Correction: that threshold is your ICB’s, not a national one. They differ and they change by year.
comment on [Question] why is the sequence written four different ways across vendors in c/bpc157 · 6 points · 5 days ago
preclinical enthusiasm is not clinical evidence
comment on 2.5 → 5 → 7.5 in six weeks was a mistake, here is what happened in c/tirzepatide · -17 points · 7 days ago
The distribution matters more than the mean. In the trial population the interquartile spread was wide enough that two honest people can have completely different runs on the same arm.
comment on [Caution] research use only. handling questions welcome, protocols are not. in c/bpc157 · 11 points · 7 days ago
Ask for the theoretical mass alongside the measured one. It is a one-line addition to a certificate and it converts a purity claim into an identity claim.
comment on [Titration] 12 weeks per step is not too slow, it is just unfashionable in c/tirzepatide · 11 points · 13 days ago
It is a dual GIP and GLP-1 receptor agonist, and the GIP arm is the part that has no equivalent in the semaglutide threads. That is why the side-effect profile reads differently rather than just milder.
comment on pentadecapeptide is the most under-discussed thing on this board in c/bpc157 · -33 points · 1 months ago
Not convinced. A high purity figure with no identity confirmation tells you the sample is homogeneous, not that it is what you ordered.
comment on [Meta] the FLOW rule is doing its job and people should stop complaining in c/trialwatch · 26 points · 1 months ago
Open-label extensions lose their randomisation. Anybody still enrolled at week 104 is a selected group and the numbers describe that group.
comment on am I the only one who found Mounjaro harder than the injections in c/tirzepatide · 55 points · 1 months ago
Week 29 was the first time the scale moved after a five-week stall. I changed nothing in that window.
comment on [PSA] PeptideMeter results are area% by default and people keep forgetting in c/tirzepatide · 0 points · 1 months ago
Yes. I stepped to 12.5 because the calendar said so, not because anything needed fixing, and I regretted it for a fortnight.
comment on [Lab] split one vial across Medutest and Janoshik — 99.3% and 98.7% in c/tirzepatide · 6 points · 2 months ago
Cosigning the four-week thing. It is a minimum interval, and treating it as a schedule to keep up with is how people end up miserable at 12.5.
comment on [Discussion] TRIUMPH is doing more work than we give it credit for in c/trialwatch · 14 points · 2 months ago
Agreed on comparators. "Superior" means nothing until you know superior to what and at what dose.
comment on how much of what we believe about endpoint actually comes from FLOW threads in c/trialwatch · 0 points · 2 months ago
Disagree — that figure is from the diabetes programme and you are quoting it as an obesity endpoint.
comment on [Discussion] we are measuring Zepbound at the wrong time and calling it noise in c/tirzepatide · 18 points · 2 months ago
This matches mine. Milder fatigue than I expected, and what there was settled inside a fortnight of each step.
comment on MHRA — 20 things I got wrong before I got it right in c/glp1uk · 53 points · 3 months ago
In England, specialist weight management services are commissioned locally, so eligibility criteria and waiting times differ by ICB. There is no single national threshold to quote.
comment on [Question] endpoint — what am I missing here in c/trialwatch · 14 points · 3 months ago
Push back: an open-label extension tells you about the people who stayed.
anders_vermeulen is right about the programme names. They are different populations with different endpoints.
comment on unpopular opinion: most of what gets said here about 15mg is guesswork in c/tirzepatide · 36 points · 3 months ago
SURMOUNT-1 ran 72 weeks with the top arm around 21% mean body weight change. SURPASS is the diabetes programme and reports glycaemic endpoints, so quoting the two interchangeably is a category error.
comment on [Results] 45 weeks, 14kg, and tirzepatide was the hard part in c/tirzepatide · 26 points · 3 months ago
Disagree with the conversion table. There is no validated equivalence between the two molecules and posting one as though there is does real damage.
comment on [Discussion] we are measuring NHS at the wrong time and calling it noise in c/glp1uk · 10 points · 6 months ago
A private prescription is an independent route: it does not affect NHS list position, and it is not a step in the same pathway.
Agreed — and keeping the letters with dates is what makes any later challenge possible.

Sent a vial to Janoshik: 98.2% against a claimed 98.0%, with the mass confirming the sequence. Both halves mattered to me.

SURPASS is the diabetes programme and reports glycaemic endpoints
comment on [Question] France — has anyone got a straight answer on UK in c/glp1uk · 2 points · 7 months ago
Not convinced — the US threads are a different system entirely and importing their advice here does real harm.

intention to treat versus completers changes the number substantially

Cardiovascular outcome trials are powered for events, not for weight, and are typically run in a different population with different inclusion criteria. Reading a weight number out of one is reading a secondary endpoint.
comment on [PSA] pentadecapeptide is not what most of this community thinks it is in c/bpc157 · 31 points · 9 months ago
Short peptides in solution are subject to hydrolysis and, depending on sequence, oxidation. Storage state and time in solution are practical variables that this board almost never discusses.
comment on [Meta] the FLOW rule is doing its job and people should stop complaining in c/trialwatch · 0 points · 9 months ago
Relative risk reduction without the baseline rate is uninterpretable. A large relative reduction on a small absolute risk is a small absolute benefit.
comment on [Discussion] TRIUMPH is doing more work than we give it credit for in c/trialwatch · 6 points · 9 months ago
Spent an evening with the appendix tables and found the subgroup detail that the entire thread had been speculating about.
comment on reading MHRA threads from 2024 and half of it aged badly in c/glp1uk · 38 points · 10 months ago
Right, and reading the actual supply notice rather than the coverage of it saves a fortnight of speculation.

a lot of people plateau nicely at 10mg and never need 15
comment on why does nobody talk about pricing in c/glp1uk · 15 points · 11 months ago
Small fix — that is the Scottish pathway. The post is asking about England and the answer is different.
comment on the STEP thing finally clicked for me and I want to write it down in c/trialwatch · 25 points · 11 months ago
The registered protocol is public. Comparing the registered primary endpoint with the reported one is a two-minute check and it is how outcome switching gets caught.

Yes — 10mg being a genuine landing place for a lot of people is underrated. There is no medal for 15.

comment on reading 15mg threads from 2024 and half of it aged badly in c/tirzepatide · 5 points · 1 year ago
Careful with "everyone tolerates it better". The people it did not suit stop posting, which makes this board look calmer than the drug is.

Have you seen the current threshold published by your ICB?

Disagree.
This is why national advice does not work here. It is commissioned locally and it varies.
