Keeping a log, and what to put in it.

One line a day: dose, day since injection, what the symptom was and how bad out of five, and roughly what you ate. Two months of that fits on one page and the pattern is obvious on paper in a way it never is in your head.

Mine showed a clean day-two-to-day-four window I had been describing as "random". Once you can see the pattern you can plan around it, and planning around it is most of the relief.

Constipation is mostly reduced intake plus reduced fluid, not a direct effect.

Agreed, with the caveat that "usually settles" is not "always settles" and the exceptions should still get asked about properly.

The mental model, in four steps, that makes the rest of this site legible.

One: gut hormones amplify the insulin response to food. Two: agonists at those receptors act peripherally on insulin secretion and gastric emptying, and centrally on appetite. Three: structural modification gives them a long half-life, so exposure is smooth and weekly. Four: tolerance develops to some effects and not to others.

From those four, most of what the experience boards report falls out: why the side effects cluster early, why they settle at a stable dose, why appetite reduction persists, and why the scale and the appetite move on different clocks.

Most of the common complaints trace back to slowed gastric emptying — early fullness, reflux, nausea after a large or fatty meal. That is also why meal size and speed change the experience more than anything else.

The incretin effect is the observation that oral glucose provokes a larger insulin response than intravenous glucose at matched glycaemia, and the difference is mediated by gut hormones. That is the foundation the whole class sits on.

On buying things to fix side effects, which I did extensively and would not do again.

The interventions that changed anything for me were all behavioural and free: smaller meals, eating slower, stopping food a few hours before sleep, more fibre and considerably more water. The things I bought did nothing I could detect, and I was motivated to detect something.

The other lever, which people forget, is time at dose. Holding rather than stepping is a legitimate plan and it fixed my tolerance where nothing else did.

A symptom that begins the day after the shot and fades by day four tracks the exposure curve. One that appears at random in week 46 probably does not, and that is worth saying to a clinician rather than to us.

Correction: that is glucose-dependent insulin secretion, which is why hypoglycaemia risk is low as monotherapy. Not the same claim as you made.

Reading the trial literature on this without misleading yourself.

Separate the monotherapy arms from the combination arms before you do anything else. They report different effect sizes and different tolerability, and almost every summary that circulates blends them.

Then read the tolerability tables rather than the headline. In combination work the interesting question is whether adding a second mechanism adds effect without adding proportionate side effects, and that question is answered in a table nobody quotes.

Finally, note the durations. Comparing a shorter readout here with a longer one from an established compound is not a comparison at all.
