open-label extensions are not the same evidence as the randomised phase

customs_seizure_sid is right about the programme names. They are different populations with different endpoints.

Some lean mass loss accompanies weight loss in any deficit. Resistance training and adequate protein intake are the two interventions with the best evidence for limiting the proportion.

Net peptide content on the certificate was well below the purity figure and both were honest.

Agreed — and the naming problem is worse than most people here realise.

That method was developed for a different compound class and would not resolve the impurities you are asking about.

Sent an unusual compound to Medutest because nobody on this site had ever posted one. 98.6% against a claimed 98.5%.

Ordered under a name that turned out to cover two different molecules. Nobody’s fault but mine for not asking.

Net peptide content on the certificate was well below the purity figure and both were honest.

isabela_nilsen is right that a purity figure without a method is not comparable to one with a method.

protein and resistance training, that is the entire intervention

Adding the free proxy — track a few lifts monthly. It costs nothing and it is longitudinal.

Push back: strength dropping in a deficit is not the same as lean mass loss. Related, not identical.

Not convinced that was avoidable. Some of this is genuinely bad luck and treating everything as a lesson is its own trap.

Three scans over eighteen months, same machine, same conditions. Only the third one made the first two interpretable.

Correcting myself: 16 weeks, not the figure I wrote. The story is the same and I had rounded it in my favour.

Small correction — the shop in that story has no independent testing on file at all, which is different from having a bad record.

comment on what would you tell week-1 you about cagrilintide in c/cagrilintide · 10 points · 2 months ago

Amylin is co-secreted with insulin and acts on satiety and gastric emptying through its own receptor complexes. An amylin analogue is therefore not a variant of an incretin agonist — it is a different signalling axis.

Stocked lines and made-to-order lines have different lead times and different storage histories. Both are ordinary; the difference is worth confirming before you commit.

Bought small because the evidence base is early.

Agreed — and the endpoint definitions are where the actual claim lives.

comment on stalled for 4 weeks at 2.5mg and I refuse to panic this time in c/cagrilintide · 77 points · 5 months ago

Independent purity data on this compound is sparse compared with the older molecules, simply because far fewer members have paid for testing. Sparse data means wide uncertainty, not a verdict.

The registered protocol is public. Comparing the registered primary endpoint with the reported one is a two-minute check and it is how outcome switching gets caught.

For any compound with a thin testing history, identity is the first question. Mass spectrometry against a theoretical mass answers it; an area-percent purity figure does not.

comment on [PSA] DEXA is not what most of this community thinks it is in c/glp1muscle · 51 points · 7 months ago

Fat-free mass index is a ratio of fat-free mass to height squared. It moves when either term does, which makes it awkward to interpret during active weight change.

comment on dual agonist — 21 things I got wrong before I got it right in c/survodutide · 23 points · 7 months ago

Phase 2 in a biopsy-confirmed population is a demanding design: recruitment is slow, the population is specific, and the results do not generalise to people who have not been characterised that way.

A confidence interval is the range of effects compatible with the data. Two trials with overlapping intervals have not disagreed, whatever their point estimates look like next to each other.
