comment on why does nobody talk about concentration in c/reconstitution · 8 points · 5 months ago

a U-100 syringe reads 100 units to the millilitre, every time

Circumference measurements often continue changing during a scale plateau because composition changes are not visible on a bathroom scale.

The incretin effect is the observation that oral glucose provokes a larger insulin response than intravenous glucose at matched glycaemia, and the difference is mediated by gut hormones. That is the foundation the whole class sits on.

My goal weight at week one was a number I made up. I have revised it twice since and I am happier for it.

comment on [Question] Ireland — has anyone got a straight answer on 503B in c/compounding · 17 points · 7 months ago

Small fix — 503B facilities register with the regulator; 503A pharmacies are licensed by the state board. Different mechanisms.

if the maths gives you a fraction of a unit, change the concentration

Kept the label from every vial. When the shortage status changed, having the paper trail made the conversation much shorter.

comment on help me understand shortage list, I have read the wiki twice in c/compounding · 16 points · 8 months ago

Reminder that nothing here is medical advice and this board cannot tell you which arrangement to choose.

Adding one thing — ask for the facility name. Most will give it, and the ones that will not have answered you.

Same. I got this wrong once, caught it because the units looked absurd, and have written on every vial since.

That is sterile water, not bacteriostatic. Same appearance, different product, and it matters for a multi-draw vial.

comment on unpopular opinion: most of what gets said here about mg/mL is guesswork in c/reconstitution · 1 points · 9 months ago

The arithmetic in full: concentration is mass over volume, so 20mg into 3mL gives 6.67mg/mL. A U-100 syringe is graduated at 100 units per millilitre, so 2mg is 2 divided by 6.67 millilitres, which is 30 units.

comment on what would you tell week-1 you about contraception in c/glp1women · 13 points · 10 months ago

That is a general statement about gastric emptying, and the question was about a specific product.

Adding the one thing nobody warns about — fertility can change, and quickly.

comment on unpopular opinion: most of what gets said here about customs is guesswork in c/customs · 95 points · 10 months ago

Correction: that is a request for information, not a seizure notice. Different document, different process, different deadline.

Have you asked the supplier what they declare?

permalink

comment on unpopular opinion: most of what gets said here about customs is guesswork in c/customs · 107 points · 10 months ago

Asked the supplier what documentation travels with the shipment and got a straight answer. That was reassuring in itself.

comment on appetite is the most under-discussed thing on this board in c/glp1science · 65 points · 11 months ago

Tolerance to the gastric effect develops with continued exposure while the appetite effect largely persists. That single fact explains most of the "it settles but it still works" pattern the side-effect board reports.

permalink

comment on [Question] how do you actually verify bacteriostatic water in c/reconstitution · 15 points · 1 year ago

Reconstituted at 3mL when I meant to use a different volume. Caught it because the units looked wrong on the barrel. Now I write 5mg/mL on the glass before I put the cap back.

Correcting my own comment: the shortage status changed in the interim, so the arrangement I described no longer applies.

Adding one thing — ask for the facility name. Most will give it, and the ones that will not have answered you.

Nothing in this thread is medical advice, and the choice between arrangements is one for you and a prescriber who knows your history.

Yes. The central component is the one that explains the reports on this site better than gastric emptying does.

do the arithmetic before you reconstitute, not while holding a syringe

permalink

comment on genuine question about customs that I am slightly embarrassed to ask in c/customs · 10 points · 1 year ago

Where a supplier holds regional stock, the shipment is domestic and there is no import event at all. That is the structural fix rather than a workaround.

Is that a U-100 syringe? The barrel markings change the answer completely.

This. Showing the working is the fastest way for somebody else to spot the mistake.

Started reconstituting everything to a round concentration and the six-in-the-morning error rate went to zero.

permalink

comment on jurisdiction — 14 things I got wrong before I got it right in c/customs · 1 points · 1 year ago

Adding the standing caveat — nothing here is legal advice and this one genuinely warrants somebody qualified.

lane_watcher is right that regional stock is the structural answer rather than a trick.

comment on someone explain mg/mL to me like I have not read a paper in years in c/reconstitution · 11 points · 1 year ago

The arithmetic in full: concentration is mass over volume, so 20mg into 2mL gives 10mg/mL. A U-100 syringe is graduated at 100 units per millilitre, so 5mg is 5 divided by 10 millilitres, which is 50 units.

GIP receptor biology is genuinely unsettled — there is a live argument about agonism versus antagonism at the receptor — and the clinical results are robust regardless, which is an uncomfortable and interesting position.

permalink

comment on [Discussion] can we stop arguing about gastric emptying until somebody posts a number in c/glp1science · 325 points · 1 year ago

GLP-1 receptor agonism acts both peripherally — insulin secretion in a glucose-dependent way, slowed gastric emptying — and centrally, on appetite regulation. The central component is the better explanation for sustained intake reduction.

I would not read the price difference as a quality signal. Most of it is the consultation model, not the vial.
